Use this form for students only please.

Completed 6th graders to Completed 12th graders

Adult Volunteer Form

Use this form for all adults attending Basecamp.

Each church is responsible to provide one qualified volunteer male adult per 6 male students and one qualified volunteer female adult per 6 female students. We have a program in place to help you meet this requirement. Click Here to read about our Sherpa Program.

Sherpa Request Page

Use the contact form to request the use of our Sherpa Program. Indicate how many and what gender leaders you are in need of. You can learn more about the sherpa program here.

Church Child Indemnity Form

This form must be submited with all other paper work prior to June 1st.

Rooming List

your group will room together by ages. Please submit a rooming list. Put 4 in a room as much as possible. It is possible that other groups may be roomed with your group. There will always be atleast two students from the same church together.
